In most organisations the servers belong to IT and the power and cooling belong to facilities, and neither group owns the interaction between them. That gap is where the failures come from and where your work sits.
Nobody owns the whole picture
IT adds equipment without knowing the remaining power or cooling headroom. Facilities maintains infrastructure without knowing what the load will be next quarter. Neither has the information the other needs, and no meeting exists where they exchange it.
The room outgrew its design
Almost every enterprise server room was designed for a load reached years ago and has been extended by adding equipment. Power density, cooling capacity and distribution were never revisited, which is why hot spots and capacity limits appear without warning.
The risk is invisible until it is not
Single points of failure, batteries past their life, generators never tested under load, and cooling with no redundancy sit quietly until an outage. Owners are usually unaware of the exposure, and quantifying it is the most saleable thing you do.

For one facility, establish who owns power, cooling and equipment installation decisions, and where those roles meet.
